Standardizing Pattern Credits

The "Authors" column contains inconsistent naming conventions (e.g., "Kazyan" and "Tanner Jacobi" both appear, despite being the same person). I propose using the table at User:77topaz/Forum handles table to determine what name to use, giving preference to the columns from left to right.
